PRODUCT DESCRIPTION INFORMATION

What is this insurance? Can you tell me more about this insurance?

Guaranteed Acceptance Life Insurance pays a lump-sum benefit upon the death of the insured. There are no medical exams or health questions. Benefits won’t decrease regardless of your age or health. Your financial institution is offering you $2,000 of coverage at no cost to you for being a valued customer.

DEPENDENT and AGE QUESTIONS

  • Insured: Must be between age 18 and 65 at time of enrollment
  • Individual coverage only (no family coverage)
  • Terminates at age 80

BENEFITS

  • Offers up to $50,000 of additional coverage elected and paid by the insured
  • Individual coverage available (no family coverage)

ADD ON BENEFITS:

  • You only need to read the description for the benefit being asked about

“You automatically receive the following benefits:

Accelerate Payment of Death Benefits – this is an option that may allow someone with a terminal illness to receive part of their life insurance benefit amount early instead of waiting for it to be paid to beneficiaries after death. Eligibility requires medical documentation and approval by the carrier, Prudential. Any amount received early under this option will reduce the benefit payable to beneficiaries later.

If insured has additional coverage: “This also includes a chronic illness acceleration feature.”

Conversion Privilege – you can exchange your additional term life insurance for permanent coverage – without a medical exam. Insurance ends at age 80.”

HYPOTHETICAL or “IS THIS COVERED” SCRIPTING

This policy does not list any specific exclusions for the life insurance benefit. In general, if an insured passes away, the benefit will be payable once Prudential receives proof of death. Keep in mind coverage ends when you reach age 80. Coverage is subject to the terms of the policy, including eligibility requirements and when coverage ends.
