Certificates CANNOT be sent via certified mail.

Duplicate Certificate Request 

“I can email you a duplicate certificate or you can go to fmservice.com and create a log-in to access your policy information.”

From the Latest Customer Coverage screen, press F12 to request a duplicate certificate for the customer.

To email the duplicate certificate, enter “Y” for Email Delivery, then follow the prompts of adding or editing the email address. Please inform the customer it can take up to 24 hours to receive the duplicate certificate via email.

Press <F8> to see when we have sent duplicate certificates to the customer in the past.

For Minnesota Life customers:  Read the following script verbatim before pressing F12.

“If you can open attachments via your email and download and/or print the attachments, I’d be happy to send you the duplicate certificate to your email address. Otherwise, I can also have a hard copy mailed to you. Which do you prefer?”  If the customer wants the duplicate certificate emailed, “If you have trouble printing or saving the duplicate certificate, call us back at the number you dialed.”

Customers can view and print their certificate of insurance on fmservice.com.
