Coverage Type or Coverage Amount Change

I’d like to add my family to my insurance.

“I can email you a form for you to change your insurance coverage.”

If sending a form, go to the F13 screen and select the AD_006 / AD006 form.

I’d like to change my insurance so it covers only myself.

“I can email you a form so you can change your insurance coverage.”

If sending a form, go to the F13 screen and select the AD_006 / AD006 form.

I’d like to cancel my supplemental / additional insurance (not the complimentary coverage).

Follow cancellation procedures.

Customers cannot change their coverage type (i.e., family to individual) on fmservice.com.

Eligible customers may receive an increase in coverage alert on fmservice.com or an increase coverage form via mail or email.
