Eligibility

• 18 or Older and a customer of the Plan Sponsor
• No termination age, however, In Hospital Accident Daily and ICU benefits reduce to 50% at age 65 and to 25% at age 75. ER benefits remain the same regardless of age.
• Acceptance is guaranteed-no medical exams or physician statements required.

Cash benefits

Hospital Accident Daily Benefit:

• If a covered person sustains an injury which results in hospital confinement within 365 days of the date of injury, the plan will pay the daily benefit selected for each day of hospital confinement. This daily benefit is payable for up to 365 days for all injuries resulting from any one accident.
• Plan pays $900 per day for each day of hospitalization- up to 365 days for covered injury

ICU Daily Benefit:

Plan pays $900 per day for each day in Intensive Care Unit- up to 365 days for covered injury

Emergency Treatment:

Plan pays $150 for emergency room treatment of a covered injury

Additional Features

60 Day “No Risk” Trial- no premium due until 60 days after the coverage effective date

Family Coverage

Spouse:

• Emergency room benefit — $150
• Spouse with children — 50% of daily hospital benefit
• Spouse without children — 60% of daily hospital benefit

Dependent Child:

• Emergency room benefit — $150
• Each dependent child — 20% of daily hospital benefit
